Pal-less Palace II not being awarded

Steps:
1. Begin new solo save file from floor 1.
2. Clear floors 1 - 100 without any KOs/deaths, in a single game session.
3. Enter floor 101.
4. Die at floor 103. (First KO on save file)
5. Game shows "Duty Failed", then scoreboard comes up.
6. After pressing exit, return to Quarrymill without Pal-less Palace II being awarded.

Prior to this, I had already attained Pal-less Palace I in a previous save file.
