Lost items and unlockable quest

Date & Time: Started when I transferred to a new server early this week. I'd say 1/15

Area and coordinates: Kugane by the MB
Housing: doesn't matter
Steps: I have lost items and the quest for Shinryu's Domain is not showing.
1. Missing Shinyru's Lance
2. Missing Augmented Lost Allagan Pantaloons of Maiming
3. Missing the Minstrels Ballad Shinryu's Domain from my duty finder.
4. Missing the quest to get it back.

Let me just specify that I have world transferred before without any issues. I transferred to Ultros to help a friend level and when I was done I figured I would transfer back to Shiva.

From what I see it has taken a earlier version of my character. Even my hot bar was different upon transferring. I had most of the Lost Allagan set of Maiming and Shinyru's Lance but they are now gone from my character and inventory. I have checked all retainers as well.

The real issue is I'm experiencing a bug where I cannot unlock Shinryu's Domain via the Wandering Minstrel in Kugane. I have the Pool of Tribute and Emanation both Extreme variants unlocked and completed but no Minstrels Ballad Shinryu's Domain. I had it unlock and completed back on Ultros, that's how I got the Shiyru's Lance. Please help get my items and quest back! Been playing everyday trying to get to max level and this is very discouraging.
