Do you fulfill all of the following requirements?

1) Must have one job from each role (Tank, DPS, and healer) at level 60 OR Have at least one class from both Gathering and Crafting at level 60, crafted 100 or more collectables, and gathered 300 or more collectables.
2) Completed 1,000 or more instanced trials and/or dungeons
3) Earned 300 or more commendations

You can't just have one job from each PvE role at 60.
