When I try the 2 or 3 phase mudras, sometimes I see lesser combinations firing out before the actual 2 or 3 combination I'm going for launching...
I couldn't find if it's an actual proc or it's just a visual bug.
Any words on this?

Lag issues. Every NIN has to deal with this and it takes forever to get used to and even those of us who been playing it for a while still mess up.

Actually isn't a mess up per se, because the desired effect, like Huton, for example, goes off, but before that I see either the shuriken or the lightning going too, but wasn't sure if they were just a visual bug caused by lag or they actually did something.
Yea that happen to me all the time, a reason i stop playing nin. Is more likely a latency problem but no other job have it that bad
you may be getting the bug where both actually happen as well, my friend got that in heavensward and it was doing both effects and damage...
Edit: i believe its caused by your system sending all the inputs at once which causes the game to register both actions
How fast are you hitting the buttons?
My best guess is this.
FF14 works a bit differently from other MMOs in the Client/Server department. Using WoW as an example, the client sends your action to the server, and then the server confirms that it can happen, then the client responds. (It's why you'll start moonwalking, or just have a highlighted button but not do anything)
FF14 the client responds first, but requires confirmation from the Server for the result. Which is why when you're 90king, you can still push buttons, move around, and see the visual result of your input. (Until it catches up, or kicks you)
So, the guess is: You do all your mudras and hit the button, but the moment you input the third, while that's being sent to the server (and the 'confirm' coming back to change your Ninjutsu icon), you've already hit your Ninjutsu. Which is why you're seeing an effect 1 step early (confirmation didn't come yet) but getting the current step effect (ninjutsu command was sent with the 3 mudras attached)
I got an awesome triple once. Fuma turned into Raiton, which then turned into Suiton. Cancelled my queued GCD twice, though, costing me roughly a second of uptime.
A hell of a lot better than casting Suiton and actually getting the Fuma and only the Fuma, though, which started happening again in SB after avoiding it since the server change in HW.
