DirectX Unexpected Error (Crash)

Steps:
This has occurred twice in Heavensward.

First time:
During the quest "An End to the Song", during the cutscene when Aymeric, Warrior of Light and Alphinaud mount the dragons and take to the air, during the middle of the cutscene I got the DirectX 11 error. I got past this point by just skipping the cutscene.

Second time:
It happened again during the quest "One Life for One World". After Urianger freed the party from the chains, and the Warrior of Darkness gives the Magus of Darkness a look, Alisaie says something and during this part I get the DirectX 11 error. This is where I currently am.

This error is a pop-up window with a bunch of text on it. The game still runs but the cutscene does not advance. When you hit "OK" on the pop-up window, the game closes. This error happened to me every time I reached the exact same spot in the cutscene, without fail. I tried it in DirectX 9 mode and it still happens - same error box but the text is different (no dx11 tag in exe name). I also tried playing the game in borderless window and full screen - still it crashes. I'll try a few more things and see if anything works after I post this.
