Gamepad vibration bug?

seems kind of random but here lately I've noticed that sometimes my xbox one controller will not be vibrating, sure enough I go into the options and the enable vibration is unticked. when I tick it latter that day it will random untick itself. Is there a trigger that causes it to auto turn off vibration or is this a bug?

I've always had bugs with the XBox Windows controllers (wired & wireless) when using vibration. I find that it either doesn't work or crashes the game. Has been that way for me since 2.x when I first tried it for fishing.
