Well, in a way it does. For chests that have the PLD sword, the shield will come with it. I believe this was a change made in 3.2-ish, but only to HW areas and beyond.
Perhaps it could be changed so that the name of the weapon + shield (if completely different), or the [name] sword and [name] shield becoming say "[name] box" - so you roll on a box, if you win it, you use it, open up the sword and shield, and away you go. Then you're only rolling on one item. The new artifact gear, as well as the i115 gear for RDM/SAM, work this way.
yeah, i can imagine 2 PLD in the party and one gets the sword and the other one the shield... they should fix that, because that's a situation that should not be possible.
I imagine a lot of people would dislike that just because they wouldn't be able to do sword or shield-specific glamours, the invisible shield especially. Maybe they could drop and be purchased as a bundle that unpacks into the individual items? Similar to the AF3 boxes.
There's certainly a precedent to combine them now we have RDM (sword + focus), and NIN (dagger + dagger), which don't use the offhand slot, despite having an offhand. The other classes that use that slot (ie crafters, gatherers) actually have a second tool in those slots, not an off-hand. WHM and BLM *technically* use that slot for shields but, there hasn't been any new BLM/WHM offhand shields since 2.0
