Steps:
I was in the process of completing the requirements listed for obtaining the Pal-Less Palace I achievement, (Clear Floors 1-50 solo).
After killing Edda Blackbosom, and subsequently exiting Floor 50, the achievement did not proc.

Achievement is awarded when your run ends, so keep going on that save file until you are defeated.

I continued on that save file from 51-60 and died to Floor 60 boss and that did not happen.
