I'm not sure what's going on, but it's definitely related to the outbreak of Roegadyn who just cannot lift that brick.
Would make for a nice quest, save a guy from hanging and he'll give you a reward. It could be the post moogle quest for HW.
The man in Coerthas actually eventually falls, but lives and just stays on his back complaining about slipping for a while!

There is a worker hanging off some scaffolding in Ishgard, too. There is a knight panicking in the ground level below him.
