PS3 to PS4 upgrade campaign questions

If I have ARR for ps3, ps4, and pc.

But Heavensward for ONLY PC and PS3..

Can I upgrade my PS3 Heavensward to PS4?

Yes you can upgrade your ps3 HW to ps4 HW for free

How do you upgrade PS3 Heavensward to PS4. When in mogstation there is no option that I can find that allows me to do that. I did the free upgrade in 2014 during that free upgrade campaign, then I needed to be able to play on ps3 again after that and added ps3 codes for ARR and Heavensward sometime after heavensward came out.
