Changing materia on gear

How can this be done? Once I have affixed the IV can I change/upgrade to V? And if yes how is it done?
Any help would be appreciated.

You can change materia, but doing so will remove and destroy all materia on the item. To remove materia take the item to Mutamix in the Bonfire in Central Thanalan. Then re-meld. Just fyi if this for already melded tomestone gear, the materia melded on the gear is destroyed when the item is upgraded so unless you have a pressing need for a Tier V meld, I'd wait until it is upgraded before re-melding it.
