Black Screen Error EVGA GTX 980

Hello, everyone

I have been playing this game for a few weeks now, but an issue has become more and more prevalent to the point where the game is now unplayable.

I can log into the game world, but soon after )anywhere from 10 minutes to immediately after) my monitor will go black, it will flip from HDMI, to VGA, ro Display Port inputs, then give a "no signal" message. The game will still be running, I can hear it and if I hit escape it give the sound of the window opening, but no image. I am forced to hard restart my PC to continue.

I have tried switching from DX11 to Dx9, fullscreen to windowed, even uninstalling and re-installing my graphic drivers, no dice.

My rig is more than capable of running this game. I have a i7 4790k, GTX 980 FTW, 16 GB of RAM and a GX 750W PSU. My friends with less powerful components have no issues, NOR does this issue happen on any other game, so I highly doubt my setup.

I am at my wits end trying fixes I've read online with no dice, so any help would be greatly appreciated, thanks.

Here are the things I tried since posting in the FFXIV subreddit
1 Trying another HDMI cord, no dice

2 Roll back GPU drivers to prior version, no dice

3 Increased fan speed with Precision X to lower heat, no dice

4 Checked Event viewer for errors, nothing in the event viewer other than my hard reset of the PC
