Cannot Progress GC rank to 1st Lt, quest does not show up, promotion not available.

Since the new Patch 3.4 came out I have not been able to progress my Grand Company Rank Beyond Second Storm Lieutenant rank. I have the right quests done, I am the correct class and should be able to take the "Squadron and Commander" quest at Maelstrom command to start the rank up process but it does not show up. I have been able to do other quests with no problem, just this one doesn't show up.

I saw that someone else had submitted this bug, but that thread got moved to "insufficient information" so I decided to resubmit with as much info as I could.

1.Quest "Rising to the Challenge" already completed.
2. Spoke to "Storm Personnel Officer" as Level 60 Dragoon in the Maelstrom Grand company, at Second Storm Lieutenant rank, and the quest "Squadron and Commander" is not available.
3.Attempting to apply for a promotion results in a response of 'The Maelstrom recognizes your service, but currently there are no further ranks available to you
