Silly thing to ask - A print of the forbidden materia craft scren

Can anyone post a print of the materia meld screen when you are melding a second materia to any gear? It does show the probability of it working, doesn't it?

Need this for the FF wiki materia page, and I do not have the augmented melder or time to get one in the near future. Tried searching a bit, but couldn't find it.

I would also love to get my hands on a print for the failure message and the gathering catalyst skill activating / that shiny effect on the aiming UI it gives, but just the materia meld screen would help a lot.
