yes ive had actually two people with the same exact name in my party once.
There's probably a unique ID associated with each character, regardless of name, so that's how it can happen.
Honestly, it's more of a human thing that you can't share names per server, so people don't get confused.
Would be interesting to know, how a macro will do its job, when you are using names as targets.

Other John Doe: "I'm not dead, moron."
Healer: "I'm raising the other John!"
Other John Doe: "Well, say that next time!"
Healer: "Both of you are 'John Doe'."
Other John Doe: "Yeah, but I'm the other John Doe."
Now, if both Johns were NIN, and the WAR asks for Goad:
WAR: "NIN, can I have Goad?"
John: "Which NIN?"
WAR: "John!"
John: "Which John?"
Other John: "Huh?"
I've also met a name-clone of mine in DF once. No issue whatsoever.
