Crashing on zone in/out and teleport

Every now and then the game crashes when changing zones; teleporting somewhere, zoning into an instance or exiting them. It's very specific to these scenarios, it never crashed elsewhere. When it happens as I'm teleporting, it'll crash when I start casting and when I log back I find myself elsewhere, the place I was teleporting to.

Anything I can do about it? I've found old references to this issue but none of the proposed solutions worked for me.
