OST Minion Code

Hello all

I was hoping someone can help me with a question about the minion code from the heavensward OST, I got the cd in the post today but the paper with the code on (I think) is in JP would anyone know how I can get the code from it?

Look on the piece of paper that was around the spine of the CD Case.

It looks something like this:
