Steps:
1. I earned the achievement "Leader of the Band I" while in a duty and did not receive my reward of "Prelude - Discoveries Orchestrion Roll".
2. Submitted in-game bug report, but there seems to be no record of it.
3. I've now had 4 or 5 contacts with in-game support and no one has replaced the missing item. They had me clear out my mailbox, I waited a day (now two), and nothing has shown up.

As the game explain in the active help window that appears after you earn your first achievement, item rewards are claimed from Jonathas in Gridania.

Wow, that was it. Thanks folks. It's hard to believe I had 4-5 in-game interactions with support and all they had to do was tell me this. They even had me delete all my in-game mail, etc. I had six items waiting for me at Jonathas. This should be in one of the in-game FAQs.
