Action Bar issue

I noticed that there was a thread with the action bar being glitched or it won't work. Same thing is happening to me, although it seems to be only one button and/or skill.

Using the keyboard or gamepad to execute the command does not work.

Platform: Windows 7 Pro x64
ISP: Time Warner
Type of Internet Connection: Cable
Internet Connection Speed: 512 kpbs upload/10 mpbs download
Date & Time: All the time
Frequency: All the time
Character Name: Fire Soul
Race: Highlander Male
World: Rabanastre
Class/Skill Rank: Pugilist/12

Area and Coordinates: everywhere
Party or Solo: Both
NPC Name:
Monster Name: Naked Mole
In-Game Time: All the time

Steps:

1. Timer is up or action hasn't even been used yet, I press a button.
2. No adverse effects to prevent using an ability.
3. Nothing happens.
4. Keep mashing button.
5. Eventually goes off or I die because it never worked.
