Character and Game System Data corruption on PS4

Been having trouble past two days where I can't play cause there's a problem with my character or game system data saves. It happens whenever I'm setting up my hot crossbar. I'll start to set a Action then it goes blue screen saying the Disc is unreadable and has to close application and eject disc. The disc is clean, hasn't really left the disk drive since I got it. I can play after deleting either or both. It's just starting to be a hassle to reset all my settings, gear sets, and hud setup over again each time. Any thing I can do to fix this?
