Exploratory Missions, what do we know?

Hey all I think it's good if we share all of our information and what we know about the Exploratory Missions here in this thread so people can review and learn about all there is to know about the Exploratory Missions. So not so much a guide but more of everything you need to know can be found here.

So 1st off, spawning Rank V mobs, what is the best way to do this? just kill Rank IV mobs over and over? And what is the indication of your close to spawning one? Dose it have to be in the right place or just kill kill kill.
