Suggestion on ff14 sound
It's not a bug. It's not a technical problem. It's nothing.
So, i'll make a SUGGESTION!
I have a bluetooth headset and my fc uses mumble. If i start mumble then the game, game crashes. Is i start the game then mumble, it kills the sound.
The only way i got it to work is to set the default playback device to "communication" instead of "stereo playback". Sound is terrible but it works.
Might i add terrible is not good enough?
Suggestion is this, i can pick my display adapter in ff14 options, why can't i pick my audio device in ff14 options? Too hard for SE?
Weak.

What if you turn off sound in FFXIV?

This sounds (apologies) like a problem with the system driving your headset. Bluetooth is really designed for monodirectional communication using reasonably low bandwidth. Yes, stereo bidirectional sound is supported but it's taxing. Think if it like a 1 litre car engine; you can rev the guts out of it to reach 110mph at 7.5k rpm but it's much happier and stable at 65mph and 4k rpm. Drivers and software behind Bluetooth just sometimes (often) can't cope, especially with multiple sources trying to be compressed and transmitted simultaneously.
I suggest using a cabled headset or one running 'wireless' rather than Bluetooth (Yes, I know Bluetooth is wireless but you know what I mean).

The problem really isn't with the bluetooth. It's a symptom.
The problem is that FF14 looks at the active audio adapter the moment the game starts, and come hell or high water THAT is the adapter it will use or it will use none at all!
I have a USB keyboard with sound output (Logitech G110), and sometimes the keyboard will lose connection to the system for a few seconds due to a loose connection in the wire. No biggie in most games, the sound comes back when the keyboard does.
Not so with FF14! According to this game, I NEVER had sound on and it definitely has no idea how to play sound unless I log out, close the game, and log back in.
To quote the OP, that's not good enough! Just ... I dunno, just have the game always use whatever Windows has as the default output device at any given moment, NOT just when the game starts.
