One-time Password App Resetting

Today is the second time that this happened and I'm not sure why. I logged in about 2 hours ago just fine. I used my One-Time Password software on my Samsung Note 3. I left the house for a while then came home to log in again. My software app started up with the Terms of Service and then went to the Enter Registration Information screen. I had to use my emergency removal password to get back into the game.

Why is this happening and how do I prevent this from happening in the future?
