Go to http://support.na.square-enix.com/ or http://support.eu.square-enix.com/ depend on your account region.
Choose FF14 ARR - Contact Us - Get support - select your category, sub category & your issue in the field.
pres next - another next - choose your medium either email, call or chat support - proceed
then ticket will be created after that.
