Item disappearing after canceling NPC transaction

Steps:
1. Speak to NPC Kakalan
2. Select Artifact DoM
3. Choose Augmented Scholar's Mortarboard
4. Right click Scholar's Attire Augmentation (3)
5. Hit Escape button
6. Choose other scholar garment to augment. I chose the pants (1) and gloves (1) next, both are successful.
7. Exit npc interaction.
8. Loss of Scholar's Attire Augmentation (1) from inventory.

I've done a Search for Item In Inventory, looked all over the place, but could find none. I started with 3 Scholar's Attire Augmentation, only used 2, and the 3rd one is now missing.
