Having something very similar to this, but in my case, it's actually causing my entire PC to completely shutdown. Only happens with this game, nothing else has been causing it. It can happen as soon as I hit start and attempt to get to the "Now Loading" screen, or it can happen at any random point during gameplay. It has not mattered for location, or how crowded an area is or any other factors to really pinpoint it. I can play for maybe 20mins, 30mins, or even an hour before it'll happen. I was lucky yesterday and managed ~4? hours or so while a few FC members were teaching me a few new dungeons I unlocked while another wanted to practice tanking. But about an hour after we were done for the night (and after helping the others with their maps), it happened again. Tried again this morning, and happened again after selecting Start, and once again about an hour into gameplay.
It started happening after the 2.55 patch, although it only happened once then (probably lucky though). After the emergency patch that came out after that, it has been far too regular, but I can't say for sure if it the emergency fix just made it worse or if my play time after then just seems like it's mostly a result of the emergency patch. Either way, it started on patch 2.55, and never had any problems like this at all before the patch. I've turned off all real-time protection in all scanners while running the game, wiped every trace of nvidia drivers/configs and reinstalled fresh drivers (an older version and the latest and left the settings at default to rule out problematic tweaks) and even closed down most other programs while gaming and it has made no difference whatsoever. I've even reset the game's settings.
Pretty much everything minus a complete uninstall/reinstall of the game (or windows). I could also try installing the game in my Windows 10 Preview setup, but if it happens there too, I ain't expecting much for support for a currently(?) unsupported OS, so....
My game has been crashing pretty regularly as well from 10 minutes in to a few hours in sometimes. I would be doing just fine and then the game will freeze for about 3 seconds or so then my screen goes black. About 10 seconds of a black screen and it goes all white. After that windows tells me that the game is not responding and asks if I would like windows to try to fix the problem. Then it just died completely and I have to start all over. This has only begun after 2.55. I'm running Windows 8.1 if that matters.
Not sure if this happens to anyone else or not, but for the longest time my game loves to crash if I hang out in Mor Dhona too long. Not exactly sure what causes that but it seems the longer I play, the more apt it is crash on me in areas where large groups of people gather. (S ranks are bad too). The frame rate will steadily get worse and worse until it finally just crashes. There are also times after I've been playing a while where I go into Syrcus Tower and the game will freeze for a few seconds after defeating the boss or trash leading to a boss and the next section opens up (Assuming it's trying to load graphics for the next section of the tower into my RAM [I have 16G of RAM]). Once I can move again and proceed to the next area I notice that some enemies have not loaded and all I can see are their names or just certain parts of enemies are showing and other parts have not loaded. I'm not sure if these two problems are connected, but it's annoying. I have to reset the game once this happens and it has tended to work just fine after a reset until this latest 2.55 update where the game just out right crashes completely without warning.
Any ideas on how I can fix either of these two problems would be a big help. Thank you.
Sounds like the old texture memory problem we've seen off and on... especially common issue in the AMD graphics cards if you guys are running one of them. Have you tried the lower textures for distant objects and LOD caching options?
Otherwise, it could be the good old cooling issues rearing their ugly heads as ambient temps are on the rise and the dust bunnies are propagating like mad (spring, pollen, etc.).
I'm currently running with an AMD Radeon R9 200 series video card. You think I may have a bad driver that is clashing with the 2.55 download? I'm pretty sure the Catalyst program has me up to date on drivers. :/
Not with the download, but an issue where memory isn't managed properly. The more people and objects around you, the worse it gets. The work-around for it was to reduce the texture load (LoD streaming cache and such) to stave off the crash for a longer time. Doesn't completely eliminate it, but it can open up the window for hours of play versus minutes.
There was a drawn out thread for it a while back, but it's gotten buried over the last month or so. AMD had some threads going at their support site about it too. We're hoping it is tied into the DX9 emulation issues we have with the game in general, and that it may go away or at least improve dramatically when the DX11 client is released. Doesn't seem like AMD is having much luck ironing out the problem on their end.

Well crud... was going to try linking in one of the old threads, but it seems the last maintenance has changed something up and I'm not seeing further than a few hundred threads back. Turn on those options. Most of us have noticed the crashes, memory leaks, and frame rate drops have gone away. If you are experiencing these issues, I would suggest trying this solution.
Thanks but I have that checked already. Game still crashes at random. Sometimes minutes into playing, but it's not the slow framerate drop type that the thread you're talking about does. I can't seem to get the game to last long enough to see if that's still a problem. My game just crashes and windows says that the game is not responding and then it just gets closed out automatically. :/

I also have a similar problem, it happens in a different way: first my screen freeze, black screen to desktop, i manually kill the FFXIV process (nothing else i can do), restart FFXIV, when the main menu pops up blue screen and PC reboot.
I played 3 hours yesterday with no crash, today it has happened 3 times :S
I've activated LOD and LOD Streaming but nothing happen.
EDIT: oh and yes, i'm also using a Radeon R9 280x, driver up to date.
Got the same problem since the last patch : game crashing for no reason after a short time of play, crashing the whole comp if I try to insist coming back.
I'm trying the LOD technic right now, but it seems SE extendd the memory leak to Nvidia cards as well. If it works, cookies for everyone :D
For info, I'm running on a GTX970 with an intel i7-5930k processor at an average temp of 42°C, all drivers up to date, so I'd like to think the problem isn't coming from my hardware.
Edit : tryed a WoD to see if it was working... game crashed after loading the zone >.<
