Fantasia disappeared after world transfer?

Hello. I processed a world transfer just recently and one of my items has disappeared. I moved my Fantasia to my inventory, logged out, and processed my transfer. When I logged in after doing the required name change, my Fantasia was missing. I understand that there is a one-time item replacement for players, and I would like to use that if possible.
