missing quest item

As the quest is in my completed log with only quest text, i don't recall when I completed it other than it was within the past 2 weeks.

Method in his Malice was the quest. i didn't notice the quest item was not received until I spoke with Gerolt (north shroud 30x 20y) then Jalzahn (north shroud 29x 19y) and of a sudden i no longer have all the items needed to complete the quest. I'm not sure where said item was lost but according to your missing items people, I was NOT the one to loose it.

I would greatly appreciate it if someone could just moogle mail me my zodiac book so i could proceed with my quest.
