lost loot in World of Darkness and other dungeons

Steps:
1. When boss is defeated loot window appears.
2. Loot "Need" (or Greed, bug happens on both) on all items. Loot Window register 99 on item "Puff of Darkness" and 56 on "Syrcus Shard".
3. Loot goes to another person who rolled 94 on minion and 54 on Syrcus Shard
4. I noticed my character name didnt appear in the chat window. But this shouldnt be an issue as I reckon that character name only appear if you succesfully got the item you looted for.
5. My girlfriend who plays in the next room though confirmed my name didnt appear in her chat window too. (like she didnt see "Olfrind rolls 99 on item The Puff Of Darkness"), but i can confirm i clicked on the loot buttons, and number 99 appeared in loot window over the item icon.
6. I also noticed that people who didnt left the instance let the time limit expire.
7. I can also confirm I succesfully obtained my gear piece from World Of Darkness (because everyone passed on loot) in the same instance this bug appeared.

I think it is a bug because the minion is not on the weekly loot but you can always loot on that. Is that correct? I mean, I always have the loot buttons active when the minion drop, even if I already obtained my weekly gear piece.

Generally speaking, this happened often lately, and almost in every dungeon I entered (be it Wanderer's Palace hard mode, The Keeper of The Lake, as well as many low level dungeons. Seems to me a random behavior). Please check this as its really annoying to loot for an item you waited for days (or weeks) and when you finally roll a high number (99 is the highest possible) the item goes to another player who rolled lower than you....
