Auto Attack stop working

So far i only notice this bug while on Pug, could happen on other but as they were 50 and had more action to mess with didn't notice it. While leveling my rank 20'ish Pug few time during Behest, Leve's, and Grinding. Auto-Attack stop working, as in i just stand their and do nothing, and yes the Action bar is flashing yellow, and i have the red sword icon, and mob is right on top of me. When using actions like light strike, WS, or Featherfoot it will resume auto-attack, but they not always ready or not enough TP so I'm just a standing getting hit, and need to go into idle and back into fight mode. This happened on multiple systems.

Engage mobs, use actions (light strike, featherfoot, victumize, ect.) randomly prevent auto-attack from working.
