Additional Information:
I first noticed this while leveling a male Roegadyn a month and a half ago but have since moved on to a different race. The issue is this, When using the Fracture ability the characters left hand is not on the weapon when other races hands are on the weapon. The male Roegadyn hand is below the weapon. A similar issue occurs with true thrust where the hand is not properly holding the weapon. I am not sure if any other abilities are causing this issue. Male Roegadyn is probably my favorite race/gender and I'd really like to play one however I am easily annoyed by bugs such as this.
