Steps:
Fight against Cerberus in the 2.5 raid World of Darkness. When Cerberus is tethered with chains (main mechanic) his mesh protrudes through his texture, see attached pictures. This issue stops when Cerberus is unchained and re-occurs once he is rechained. No macros were being used and the character did not have any status effects required for the fight mechanics (Miniature).

I just thought those represented Binds, which is why they are the same color as the light/neon-green on the Pads and such.
It does kinda look like they just follow his wire-frame though if you look at it like that...
