Teleport to Aleport -> Crash

Hey there, right after 2.5 hit I'm having a lot of issues while trying to teleport to Aleport/Swiftperch during hunts.

I.E: Someone in my hunt LS calls Nahn, I try to warp to Aleport and during the teleport I have a massive FPS drop, and 99% of the times as soon as I get the loading screen the game crashes ("FFXIV has encountered an unexpected error ecc").

I haven't had this issue for a while, but it has always happened during hunts even back in 2.3. Anything I can do to fix this?

RAM: 2x4GB Corsair Vengeance LP @1600MHz
GPU: Sapphire Vapor-X R9 290 4GB
HDD: Samsung SSD 840 128GB + WD Caviar Green 1TB625W
Audio: Creative SoundBlaster Zx w/ AKG K551

I currently have AMD Omega Drivers 14.12 WHQL installed.
