Quake me up no clear.

Monday night I finally got though Titan EX. As I left the arena I noticed I didn't receive the quest completion status to conclude "Quake me up before O'ghomono" I have the achievement, quest was active, entered through DF. No clear credit was given.

Were you dead or alive when Titan died?

I was alive and very proud i survived XD

In that case I don't see why it didn't register.
