Frequency: Happens in every guild heists, fate etc (basically all content with a description above the task list (in battle)

Class/Level: So far all classes and levels (tried it with different ones)
Party or solo: happens in party and doing content solo
Area and coordinates: see freequency
Steps: start a guild heist, fate or content where u are in battle and have a description above ur task list

I am playing on PS4 with a mouse and keyboard. My setup didnt change since the PS4 release. I already tried fixing the bug by reinstalling the game to my PS4.

Internet-connection: ADSL2 16mbit downstream 2mbit upstream hosted by Deutsche Telekom
happens with Treasure Maps as well

Area and Coordinates: Happens at various locations (tested with 6 yesterday).

Steps: Open a map, defeat monsters.
