Randomly Freezing Tab out tab in

I'm really getting tired of dying every time this happens and I mean consistently.

I play game, game decides to freeze, black screen, tabs out, then tabs back in, then back to game, working again.

Only to have it happen again after 3 to 5 minutes.

And please I want a real solution and not more questions about checking my drivers and what not cause I already did it multiple times to avoid them.

But just for the heck of it here's my spec

CPU: AMD Phenom II x2
GPU: AMD ASUS R7 260x 2GB
RAM: 16GB
HDD: 1TB
MOBO: Salvaged HP Pavilion p6320f
Power Supply: 850w
Tower Case: Master Cooler HAF932
OS: Windows 7 64bit
