In the early stages of the fight before Divebombs, she usually does two Plummets before casting Death Sentence, so when I get that second Plummet, I stop my rotation momentarily so that I'm ready to pop Inner Beast. Keep an eye out for the cast bar, and as soon as you see it, pop Inner Beast.
Timings in the Twister phase become a bit more difficult to keep up with, as her attacks can become queued up behind one another, but you will always have enough time to build five stacks of Wrath in time for it, so if you're not confident, slowly cycle through combo abilities while she's animation locked with each attack to maintain your Wrath stacks, and then pop Inner Beast as soon as you see the DS Cast Bar appear.
In the final stage, she will do two phases of Liquid Hell before casting DS, so it's easy to predict when it's coming here too. In this stage I will always use a second buff, along with Inner Beast in case the Healers are being targeted by Liquid Hells and having to move shortly after DS hits.
Hope that helps :)
its cast roughly every 30 seconds.. from the very beginning, I actually run in with the off tank on the pull, heavy swing, skull sunder, (provoke) butchers block, start moving to position, pop foresight while moving, when in position(this is outta whack, cuz you won't actually finish the next combo), heavy swing (infuriate) maim, inner beast (death sentence will hit right after).. then as a little timing reminder ill hit her with fracture (30 seconds, so when it ends, is right about another death sentence). I usually have time enough for SP, BB, SP, BB combo (if you use fracture, you won't have time for the actual BB move, and you'll be using IB as death sentence hits, which does no good) if you don't use fracture, you can finish the combo, hit inner beast for death sentence. and popping a CD after the 2nd SP combo leaves it up for the hit as well.
I usually do this, after DS I count 10-11 GCD attacks (~25-27.5 seconds) then stop hitting GCD effects while waiting for DS cast bar, hit IB when he starts casting. It's essentially the same thing as timing it, but I just find it easier to think in GCDs as opposed to actual time.
Note that this isn't an exact method as using other abilities can delay his DS, however most of his abilities take about the same amount of time to go off as your GCD so you can easily sneak in another GCD attack to keep wrath stacks up. Just have to get comfortable with it, this will happen a lot more often in the final phases.
You can use Vengeance and/or Thrill for the transition from snake phase to twister while you re-time DS.
One thing I forgot to add, which since the Echo Buff, I've seen very few Main Tanks do, is that during the Conflag stage, if they're timed well, you can jump inside along with the DPS/Healers who have been targeted, and completely avoid Death Sentence (and it's healing received debuff).
Before the Echo Buff this was essential to surviving this stage, especially if one or both of your Healers had to go inside the Conflag, and I still do it even now when I'm MT.
As a rule though, you should only go in when you have enough HP to be able to comfortably survive the DOT from the Conflag (your White Mage should be keeping Regen on you), as well as Twin's subsequent Plummets/Auto-Attacks when you're targetable again.
Healers should cast SS and Aldo to counter death sentence, and E4E when is available.
I use a timer Macro that I hit every time I see the DS cast bar.
/wait 30
/p Death Sentence coming in 5 sec. Stoneskin + Adlo Please! <se.9>
Basically, once I hear the <se.9>, I do one more GCD and wait for DS to pop IB. This also alerts your healers and give them time to top you and cast Stoneskin + Adlo.
Works like a charm.
Now in between, I'd do BB > SP > BB > SP to keep the 10% damage debuff all the time.
As for the divebombs, I use Bloodbath when the Snake spawns as a 30 sec timer.
I like my rotation for it, I combo bb, sp then se then fracture and my stun to waste time. It seems around 11 hits but I macro my combos. After butchers combo I do storms path hoping it lasts to stack, then I use storms eye to buff my IB. This helps me a little bit. Usually I cast vengeance right when I get twin as a precaution then use my wrath buff to pop IB back up. If you do this vengeance is back for the first move. You get around a combo in before she hits the first but that varies on how you pull. Your fist combo watch hard for ds then run three combos back to back and wait for ds and it'll come
