I bought FFXIV disk for PS4 thinking my teens could play it. It seems that only one person can play per disk. Is this correct? I only have the one registration code. Do I buy another code or do I have to buy a disk for each player?

Only one registration code per PSN account, so yes if you don't want them playing on your ID they have to register their own game and also subscription.
What I suggest though is just let them create their own character on your user :)
