2 copies activated on PS3?

My friend bought FF14 on PS3 at launch but only played for a few weeks because she was busy with other things.
Now she wants to go back and play it again but she lost her phone with the security token app installed and she's forgotten her password and even the email address she used (she was having hacking problems at the time so started using several email addresses and passwords for different things).

I suggested that she should just buy the game again and create a new account because she didn't play much and she could buy the game for the same price as a 30 day subscription anyway.

Originally I thought she would be able to just create a new Square Enix account on her PC and then activate the PS3 version on the Mog Station but after looking into it, it seems she would need to activate the game on her PSN and then link a Square Enix account to PSN.

Will she be able to activate another copy of the game/link another Square Enix account to the same PSN account?
If not Is there a way to unlink her old Square Enix account?
Could she create a new PSN account, activate the game/link the account on there but then log in and play FF14 with that Square Enix account on her normal PSN account?
