Steps:
Hunting Log. I had cast Miasma, Bio II and Bio on my target. I went and cast Aetherflow while continuously clicking Fester for an instant kill. When Aetherflow was activated, no stacks were given and thus no Aetherflow abilities were available. No macros were used. Straight ability clicks.

Next day, I fought All-Seeing Eye in Dzemael Darkhold. I went through basic rotation, casting Bio II, then Miasma, then Bio followed by Contagion. I then clicked on Aetherflow while clicking on Fester to trigger it right away. Again, no Aetherflow stacks were given and Aetherflow itself was put on cooldown. No macros were used. Straight ability clicks.
