Game Client Freezing

My Game client is freezing when someone uses a melee limit break or shortly after that in the boss death animation. On top of the game client freezing, the windows process is unable to end. When I go into task manager to end the process, nothing happens and even ending the process tree doesn't work. I've never had any issues before, and only recently started happening after the latest patch.

In order to log back into the game I must wait an extended period of time (I can only assume for the server time-out).

This is causing me serious problems with running CT, and any boss that drops gear as I am unable to log back in by the time the loot timer has gone through. More importantly, I am useless after I or someone else uses Limit Break as my game is frozen and I am unable to participate in the rest of the fight.

I first noticed this happening when I was recently running Garuda Extreme mode. We got garuda to low health, I used the Tier 3 LB and my game froze. I couldn't log in for many min. After my game crashing after every single melee Limit break I was able to put it together.

Things I've tried:
re-installing Final Fantasy XIV.
Moving Final Fantasy to another disk drive.
updating my graphics drivers / trying different versions with clean installs
Lowering graphics settings drastically
setting all spell effects to None

I'm having no issues with other games, or performance issues on my machine.
