Questions about Free Company Recruitment Contest Rules

I am currently working on a submission and I am unsure if it follows the rules of the contest. Could someone clarify a few things?

Regarding the rules "Please use only in-game footage" and "You may not combine or synchronize the Materials with third party content," do these rules apply to title cards that we made ourselves (such as a static image with text like our Free Company name, information, and crest)? If a title card is allowed, is it OK to display it briefly while music available to us by the Materials Usage License plays as long as it's a part of our video submission?

Regarding the rule "If you use any copyrighted Materials (such as screenshots or gameplay video), then you must include the following notice," is placing the notice in the description section of the YouTube video sufficient, or should we have it visible on the video at all times?
