Crafting Random Number gerator is broken or action tooltips are incorrect

(Note to readers: Even I have defended this in the past, stating random numbers are random, but this has reached such a level of absurdity I'm convinced there is something wrong here)

Steps:
1) Activate any action which states in the action tooltip "Success rate: 90%"
2) Repeat this behavior a large number of times
3) Find success rate to be far lower than 90%

I have failed synth after synth after synth and/or failed to HQ with great frequency due to these actions not working as advertised. For every 100 actions I perform, I fail around 25 of them. Yes I know there are actions to incease success rate, but I should still be seeing much better rates than I am seeing. This has gone on for so long that I'm convinced there is actually something wrong and I'd like the dev team to examine this to see whether or not it is working as intended.
