I think that graphic is just trying to make clear that you don't have to choose which reward you want and select the sub length tied to it, but that if you choose the 90 day you get these items (and the 30 and 60 day items) and so on, seeing as how you can have only one set of black cascadier gear and one black chocobo , wind up pointer per character and they are not tradeable or sellable

Yep I got all the items on the 6th October, had my makeover and now Im the cutest cat on the server :D
I received two Fantasias. I'm not sure why. I think I was charged twice by SE. I'm not looking into it because it's only $14 and I don't want to deal with the CSRs. I'll look into it if they do it again next month though.
