I was just in Coil T4 with the worst group ever.
Right away, the dragoon said something akin to "healers, carry our FC". I saw that the other healer, a WHM, was someone I've run with before so I thought we would be fine. That makes six people who were in the same FC together plus two PUG healers (myself and other WHM).
First try: we made it to phase 3 before the dread and bugs and other guys smacked the other healer and then me to death. The FC people admit that some of them (including the tank) are first timers but it's ok because they got it.
Second try: Phase 4+5+6 adds accumulated and smacked the healer and then me to death.
Third try: Phase 4+5+6 adds accumulated, smacking other healer and myself around, and the tanks were going down so quickly, even a constant supply of cure 2's couldn't keep them up. Plus the DPS were also going down.
After the third try, these FC people said it was all our (us healers) fault when a lot of them were obviously new as they didn't know the positioning at all. Their DPS was so low that 3 phases piled up. Only one tank bothered to pull the adds off of us.
I told them to stop blaming us just because we weren't in their FC. Contrary to your belief, you all being in an FC together does not make you pro's. Then they started insulting the other WHM's abilities and his gear, accusing him of being ilvl70. (I don't think you can even get into T4 with i70?!) He was i86 and apparently, i86 is too low for T4 because they are all i100! Even though T4 drops are i90! And obviously, that i100 wasn't helping them since we had THREE phases overlap!
After some arguing back and forth, they kicked the other WHM and then (thank goodness) they kicked me too.
FC mentality: we're all in the same FC so we're all perfect pro players - it HAS to be the PUGger's faults!
I also have a horror story from Brayflox, as a level 32 SCH doing it to level up.
The first thing I notice is that the PLD tank has less HP than our DRG. We pulled the first set of mobs and I pull healing aggro and die. Same with the second pull. The tank used flash on the third pull so I managed to survive that. When the first boss came up, I was spam healing him just to keep him up. He was wearing a random mix of tank, DD, and caster gear.
Then my fairy pulled healing aggro and died. Later I have to spam heal the tank again, draw aggro, then the tank dies with a mob attacking me.
Eventually we manage to get to the final boss. The tank fails to dodge the dragon's breath and dies. After a few wipes he manages to avoid them, but has trouble keeping both himself and the dragon out of the poison puddles. After spending 30 minutes trying we manage to get the thing down, though not without me needing to resurrect the tank due to an AoE. This entire ordeal took about 70 minutes.

Tank was undergeared, we wiped several times and I kept on running out of mana healing him. That was made worse by a dps (lancer!) who was obviously not on job and therefore had low dps output.
Eventually tank & I we found a good pace and we worked together well with zero wipes. Lancer was ridiculously uncooperative. He wouldn't LB, attacked different targets from tank and did the exact opposite of what the party did (waking sleeping mobs, not getting people out of bubbles etc).
We wiped at final boss because I ran oom and had my xether on cd as he ignored our pleas to LB. When we asked if he knew how to lb or not he said "tank sux". We agreed to vote kick him and ironically enough we we joined by a proper dragoon and the battle ended fast.
I wonder how that would work out, cause you have to kill at least the one with the key, and locking out the whole other bunch.. i guess chances are about 0.6% that you manage to keep all out ^^...
And I see how Tanks Health drops with 2-3 Mobs in his back and the boss, so i am pretty sure this can´t be healed with the full first room....
However... it was not quite a "horror" story, but more a honorable mention... i did low roulette with my about 35 SCH and got Satasha with some I suppose new guys. At least the Tank was either a bot or had no idea.
I have to count on his side, he at least know that Flashlight exists... but that´s for that, no aggro management at all.
It then started at the first Mini-Boss encounter, while he couldn´t hold aggro, he just went for the Door and wanted to get through...
Short before the second Mini-Boss, he ran up to the next trash while we were still handling 3 Mobs... concerned more about my 2 DPS, sure the Tank died of not being near to us... waited,... and went offline...
We finished the Dungeon, slower than usual, but without any problems, got 2 Votes and big thx to my (IMO not good) healing skills ^^ i handed them forth to my superior fairy ^^...
SCH with Fairies in Low Levels are so OP... most of the time i forget im a healer at all ^^
It happens, my controller has died mid fight too and the team has wiped.
But you know what?

Anyone that gives you crap for 1 wipe is an ahole.
Even 3x wipes, if you are trying and polite, you don't take any crap from them. You've got a hard job and they are lucky to have you there trying to do it.
Evidenced by the fact that any mage can que for anything in about 3 seconds flat. Few want to do it, but everyone is ready to criticize at the drop of a hat.

After reading this I think I've been quite lucky with DF. I've had a few groups where people just run off or the tank thinks he's a dps which ends with me tanking most of the mobs but at least so far I've not had people shout abuse at me for not fixing their mistakes.
Probably my worst horror stories so far are Titan where due to lag i couldn't avoid the landslide. It looked like i was well out of aoe but I still got chucked off and then the other players said I was still in it. After that I was so nervous and just panicked. I was lucky to only get caught by the landslide at the very end, tanks also dies but dps manage to finish. Have been avoiding it ever since.
The other one was Cutters Cry. Me and one of the dps are new say so at the start and Tank and other fps are very patient and explaining the mechanics. We wipe a few times but it's all good. On the final boss I feel like am getting the hang of it and all is going smooth until the new dps gets the orb and runs straight at me am distracted as I'm casting cure on the tank, get hit by orb straight away the eyes glow violet and I don't make it in time to the safe area and bite the dust, felt like such a derp. Spend the rest of the fight apologising to the group.

Was doing The Lost City of Amdapor where I had my first experience with a Tank who didn't bother to stagger their speedrunning strategy for the fetid fluid sections. She would just run right through them, taking damage and pulling three or more mobs at once. Nothing the party couldn't handle as a whole, but none of us apparently had seen a tank not kill the Gadflies out of the poison, and then press on. So, I had stopped, started aiding in DPS when suddenly we realized the Tank was nowhere to be found and dangerously low on HP. Thankfully I still had her highlighted and noticed she ran ahead and just barely got a Lustrate on her in time to keep her from dying.
Would have been fine from there except she had an utter flipout moment, saying I refused to heal her on purpose and the whole party was conspiring to kill her, etc... We just let her rant for a minute before we finished the dungeon. Was more funny, than terrifying though. :P
