It's been suggested before, but being able to report people for RMT activity just by targeting them would be very helpful. It's very common to see groups of identically-dressed characters with nonsense names teleporting around zones with a modified client, killing mobs and farming materials. Even if they're not directly involved with RMT (though it's pretty obvious they are), the current means of reporting them is entirely inadequate because you have to open a ticket--and by the time the interface opens and you have to put down the characters' names, they've teleported away. Being able to report them while targeting them would go a long way towards fixing this issue.

I'd love to have a new Macro option for displaying spell information when hovering over a macro.
For example, I use mouseover macros for spells like "Nature's Minne" on Bard, but I can't display the spell's description by hovering my mouse over the macro.
We already have "/micon <name>" to change the icon of a macro.
Something like "/minfo <name>" or "/mhoverinfo <name>" would be very helpful for quickly looking up what a skill is for, without needing to put the skill itself on a hotbar aswell.

I'm not sure if it qualifies as a "QoL improvement..." No actually I think it does- I wish they would make EX mount trade-ins for 99 totems available the same patch they are released instead of 2 EX trials later.
There's little to no reason to make people who have 99 or more totems have to wait all the way until like half a year later to purchase an EX trial's mount. They essentially have proved to be able to do a trial backwards ad forwards and unfortunately have bad luck. The only argument for it is that the devs are banking on random reward design/variable ratio to keep a fairly small amount of players hooked in hopes that "this 300th kill is gonna be the one." Thus, in some misguided way the EX trial is "relevant" longer than in aught to be. The fight ceases being challenging and becomes a 10-15 minute skinner box at 99+ kills so I don't think it's particularly fair (or healthy) to not reward people who put up with it that many times during the same patch it was released.

It would be pretty nice to have an autoplay option when viewing cutscenes in the Unending Journey. Sometimes I just want to watch a whole quest's sequence and write, and having to continue to the next one manually is frustrating. Maybe make it a checkbox, like the Reflect Quest Progression option?

The Timers menu should show when your next retainer finishes a venture, without having to click the button to see the status of all of them. A lot of people are sending retainers on ventures all at once for the same time, so it would give useful information at a glance, while still allowing you to click it to get information on all of them separately.

It's probably been requested before, but I would really like to see a notification for friends on your friend list comes online... Not just for FC members ^^

Small QoL Improvements
Just a few small QoL improvements:
Expansion of the companion System, possibly linked in with Beastmaster? if it ever comes.
Ability to search mailbox for friends rather than scrolling down the list all the time.
Ability to save some favorites in the mailbox for people you send items too often.
Mentor System overhaul most unlock mentor roulette as an additional roulette so they can spam on a daily basis but a lot are not active in mentor chat.
Ishgard restoration given the same hand-in system as the system for Yellow/Whitescript.
Open world system revamp, current fate system beyond leveling and grinding for relics has no real purpose, add the Bosja based system to the open world to revamp it and add glamours to the system to give incentive + other rewards for players to work towards between patches.
Glamour System, Each class having its own glamour, e.g. Summoner and Blackmage separated so that you can have unique glamours per job.
Something to be written on the tooltip of an item so that you know you have purchased it before.
Ability to link the crafting recipe in chat, so that you can share it with a person looking for the items.
All I can think of right now.

Also, the ability to right-click materia in your menu and open the meld UI from there.

Something that would help immensely with the HUD UI is when you split your buffs/debuffs, a 3rd category that would list only raid wide buffs like Trick Attack, Cards, Technical Step, etc.
As it its right now, you can split between "maintenance" buffs like the food one, and the rest... But the problem is that temporary raid buffs often get mixed in between a few others like for example Zadnor's buffs, or shields/regens. By separating, you could place the raid wide buffs in a more central portion of your screen, without cluttering too much, since they only appear every 60s/2min.

I'd like to suggest:
1. Adding a visual indicator for each job's major cooldowns For example, Tank's mitigation and invulnerability, WHM's Benediction, DPS's mitigation and raid buffs
2. Letting players "Ping" their abilities to tell party members they are on cooldown I came up with the idea from MOBA games and thought it would be a godsend in FFXIV high-end duties, doing so in said games would display a message in chat telling others if a certain ability is currently on cooldown or not and how long until it will be ready
The reason for the suggestions is that I used to clear Savages with PuG and recently joined a temporal static, after that experience, I feel that it's a totally different game with and without communication.
In PuG, we can't communicate as quickly and clearly as through voice channel in static. There isn't enough time to type during a fight, so I often have to guess how party members are going to solve the next mechanic, are they going invul or not, are they tank-swapping or not etc.
As there tend to be numerous accidents in high-end duties, things don't always go as pre-fight discussed, that's where timely communication is in great need to come back from a dire situation.
And it's why I'd like to suggest adding tools for PuG to communicate more easily, I think it would make high-end duties a lot more enjoyable for those not in a static.
