I understand that SE does occasionally hardware and IP ban, and can ban via payment method, but these things are reserved for utterly egregious ToS violations that nothing else is working on. It's for a player who repeatedly, over and over and over, gets banned again and again, so that normal account bans obviously aren't working. When SE does this, it's not US who wants to erase a person from being here at all, it's Square Enix. And Square Enix absolutely has that right, to utterly and completely eject a customer from being a customer at all. Some people really are so badly behaved that rejecting their custom permanently is the right choice. If Square Enix has gone so far as to terminate someone's account multiple times (and that is 100% not their first choice to punish misbehavior!), Square Enix does not want that person here.
SE probably doesn't do this often, though, and it's probably something that happens more to the people who get caught up in the ban waves for RMT and botting rather than for people who get banned because they were jerks online too often.
