how often have i seen the AST makes his bubble over the party and i think: "hey, it's just right there, i can slip under it, grab a HoT and be back without rotating the boss (too much)." and in the moment i reach the bubble the AST puts it down and starts casting some other stuff... yeah, i just don't care anymore.
of course i will do 1 or 2 steps forward when there is a bubble. but i won't change my position too much. staying too close to the middle of the hitbox of a mob will quickly resolve in a 90 degree rotation, even when i think i am staying still in front of it...
i understand that an AST doesn't want to stay in front of the boss just to place the bubble over the tank. but when a WHM or SCH doesn't place it over me, i think it's not meant for me *shrugs*
Tanks that annoy the heck outta me.

As a caster what really annoys me is tanks seem to stay in place for a bit, ok cool then as casting (something with a longer cast time) the tank decides to run off and go somewheres else. And yes there is still plenty of hp left on said enemies... And many tanks do this because they are very indecisive. That stuff pisses me off, lol

Tanks that wait till mobs are half dead to pull the next pack. Super annoying, screws up everybody's rotation, and wastes CDs. As others have said, if it takes you more than a couple GCDs at most, you're doing it wrong. As an extension of this, tanks not new to expert dungeons that pull the first pack of mobs at the start, wait till everybody starts blowing up the mobs, then run to the wall. Don't be that bad tank. you're basically trolling the group at that point.
DRKs that complain excessively about stuns. Seriously, just wait a few seconds until the stun resist kicks in to use those CDs. I'm not going to sacrifice that much burst DPS and free mitigation on big pulls because you think you can do more.
Tanks that actively avoid good bubbles that are placed properly. Happens a lot. -

I've tanked plenty of times pre-Stormblood while IN THE BOSS'S HITBOX, unless they've changed the way that works, I see no reason why the tank can't step into the hitbox.

As tank I have learned to identify healer bubbles and instinctively run towards them while thinking "YAY HEALER HUGS".
Sometimes they end up in odd places, though. AST's has to be most annoying just because it's small and centered on themselves, and I have to make sure I don't move in close enough that the boss flips around to start hitting them.
As for stopping and starting when pulling, I usually have to position myself for the conal Overpower as WAR, and as PLD Flash has a slight delay so I usually pause for about a second in the middle of a pack to cast it before I move on, just to make sure I'm hitting everything in one go. I'm occasionally guilty of stopping for a bit before moving to the next pack...just because sometimes I'm in a dungeon I haven't done in a while and I just remembered there's another pack I can pick up + I decide the healer can probably handle it XD -

The WHM bubble is invisible at the limited battle effects setting. If you don't place it on the tank then don't expect it to hit the tank. The other healing AoE's should be easy to adjust to. As an AST the most annoying thing is stacking perfectly with the tank and launching the bubble only to see the tank run away from you (and the bubble) in an attempt to face the mobs away.
Edit. I just checked and the WHM bubble does show up on the limited setting. It's the "off" setting that completely hides both the WHM and SCH bubbles. -

The game really could use some better "player-casted aoe" telegraphs so that players know to step in them during the casting phase instead of running away from them. Like casting Cure III is one of those, where you really have no idea if it hit anyone because the target was moving.
