Had something like this recently in DF - however, it was a miracle, rather than the expected wipe like you had.
Someone killed the spiny WAY too early. It's shield booped and we knew we were done for. Luckily, one of the tanks used the tank LB and we managed to survive to continue.
You would think, after that + a load of "OMG", "Don't touch Spiny!", "GOODBYE CRUEL WORLD!" and "Who did that??" after Spiny went down, that the message would have gotten across. Nope.
It happened AGAIN at the second plume ADDs phase, and spiny hit the floor prematurely.
No tank LB, BUT the DPS was high and we managed to slap the snot out of her fast enough that we downed her right before her next jump/our deaths. Yeah, I don't even...
Oddly enough, it wasn't touched during the sisters/whirlwinds phase, so I'm inclined to say this was on purpose - or lucky that the offender was too pre-occupied.
Eleven and a half minute Titan HM with the only two people left standing at the end was myself (SCH) and a BLM. One DPS and the OT landslide'd off the edge by P3 and a second DPS knocked off in P5. New SMN kept dying to mechanics. Barely ecked out Heart kill and then the WHM died about three times during the course of P5 - probably on tilt after the first death because he was making some horrible movement mistakes despite playing the first 8 minutes rather flawlessly. BLM / WHM kept getting Goal'd, which allowed me free reign to break them out via SCH DPS. I will admit I fudged up on the 2nd last Mountain Buster and made a bad play as I raised the WHM and not protected the PLD in time.
Funny though, I dodged the final Mountain Buster at 1% before we killed Titan.
All this talk of Garuda and Titan EX makes me think I should just queue up for them and get it over with lol. I'm always worried that I might be a weak link because I've never done the fight before, though I do a bunch of research before hand and I try to focus as much as possible on only the things I need to do (hold back on DPS in favour of focusing more on healing and dodging). I'm just worried people will be mean since I'm new to the fight. :( (Sorry, no horror story. xP)
Garuda EX is actually super easy once you know what to expect :) (Only the people are are the unknown/chaotic element to that fight). Just remember to stay in the center for the first phase, make a break for the rock in front of you when she jumps (I like to sit right in the corner created by the rock and the wind wall.... just don't touch the wind wall). First phase with rocks you can HOly your heart out if you want - no Spiny; only Satin ^^
When you do hit the Spiny portion, don't jump in right away after the spiny breaks (I see this happen a lot with new people, regardless that they've seen a vid - just jumping the gun out of nervousness/apprehension, I guess) and when Garuda goes to do her attack while you're int he barrier, precast Medica like in Titan HM when you anticipate his landing. It'll heal people up so you can make a mad dash for the side of the room (if your group marks, it's usually Point A on the leftish side, to be considered the 'Ranged Safe place'). It helps you avoid the falling feathers that come straight after. If you get hit by them, then no biggie, just have to Esuna off her DoT.
From the safe range point, you'll be able to heal both tanks (you may have to move up into the corner of the safe crevice if you need to reach one or the other). Don't bother trying to cross the windtunnels unless you have to, they're a hassle to time (for me, anyway). z_z When she jumps again after the sisters are beaten, the churning winds will dissipate and let you head towards Spiny... then it's just rinse and repeat :D
She's super easy. I was half asleep though a good chunk of the times I queued up for her if it's any indication of her ease. xD; Just need a party who won't stand in everything or continously kill spiny x.x

I'm the same way! I get really nervous that I'll hold the whole party back and they'll be mean when I mess up (Yea, I know I have a really thin skin lol, I can't help it!). I fell behind when the Ex Primals first came out, and I haven't been able to catch up. I did do Garuda Ex a long time ago though, and decided to try it again this past weekend to hopefully get a pony. It was really easy, and even though I didn't remember what to expect, I didn't die. If you do wipe in there, it probably won't be your fault lol. So get in there and do it! :)
Titan Ex though, I hate it! I feel like I could do it if I had a ton of practice...I just don't want to waste people's time, and I assume making a learning party would be a waste of time.
Oh, umm horror story...Lots of tanks lately pulling a ton of stuff and taking so much damage, I can't really dps...Oh and they're always almost dead by the time they stop pulling, even with EfaE, Adlo, and SS. I normally play SCH, but I'd like to play WHM more, I just don't think I could handle that much damage that fast. If anyone has any tips, it would be greatly appreciated! :D
Do you mean healing big pulls on SCH, or on WHM? I can't really help with advice on SCH, being I'm rather novice/uncomfy at SCH. x.x;;
As WHM, the high cure IIs make it easier to heal through big pulls. If you're fearful of the size of a pull, wait till you recognize that the tank is about to stop (preferable if you can identify when a few seconds before the tank stops pulling) and hit Divine Seal. This will boost your healing output by quite a bit. Heal that sucker till they're topped off and watch the amount of damage the tank is taking. If you feel they'll survive a rotation-length of time, slap em with a Regen and continue healing until they seem to hold their own. You can probably get away with Cure (rather than Cure II) in that case (with Divine Seal) - just depends on how much damage they're taking. With a divined Regen, it should help tons! (think of it as constantly ticking cure Is).
On top of that, if Divine Seal is on CD and shit is still hitting the fans, a Bene will come to the rescue ^^ Just try to use it before they're too low or it won't save anything ._. (because of the slight animation delay)
EX primals are really not hard especially now with echo even a bad group can steamroll it if people don't heavy derp. The good thing about these fights and pretty much every single fight in FFXIV that is a raid they are 100% consistant. There are some tiny RNG elements like what those damn Moogles do in Mog EX and when Shiva freezes the floor etc but you can pretty much know what is coming next all the time. Garuda is super simple to heal. Stand in middle DPS for first 10% hide behind rocks for attack go back to middle holy down plumes starting with satin plume. Takes like 2-3 to get them all shroud after. Then she summons her sisters OT takes green one hopefully put divine seal regen on both tanks and virus on garuda or red add if it hasn't already been done to protect the MT. DPS green add then red one wail on Garuda some more she will go to middle and do her ultimate.
Puple tornado forms stand in middle DO NOT TOUCH THE SPINY PLUME. I like to divine seal medica II everyone and regen both Tanks the spiny will be provoked so you should get aggro from all over plumes drag them to the other side of the tornado from spiny and holy them down. She vanishes to do her ultimate again spiny plume is killed watch out for her falling feathers and the spiny explosion. Get in the shield the explosion makes and wait for her to finish her ultimate. She will summon tornados around the floor and her sisters again and the MT and OT go to different areas Healers and ranged have their own spot which is roughly behind the bottom left pillar at the start of the fight it won't be there but if you have that spot in your memory you'll be okay. Sisters are burned down ignore spiny it's killed when Garuda vanishes run and get in the shield after her ultimate back to middle for purple tornado and it just repeats over again til she dies.
That's the fight every single time nothing ever changes.
Chrysalis is horrible when tanks fight for aggro all the time. No freaking debuffs on the tanks, why tank swap on Triple Spark? Bleh.
A brilliant way to do the first trial roulette ever. Guess I'll have to heal Titan HM on the next spin x.x
Ah, sorry I wasn't clear, I was talking about speedrunning as a Whm. And thanks for the tips! I love Divine Seal, but I can't tell you how many times I've used Bene a little too late lol, I'll get that down with more practice, I'm sure. I have a couple more questions if you don't mind. If you're planning on Holying all the things, would you also use Medica 2 along with Regen? Swiftcast it, or Swiftcast Holy? Thank you again for taking the time to answer my noob questions!

In regards to Bene - I don't know if you play on PC or PS - if you play on PC then this might help a little: If you're still struggling to keep up HP or you can just intuitively tell that your next cure II will probably take too long to cast, hover your cursor over Bene - So you're still able to spam Cure/II readily but have Bene "queued up" with a click of the button. A little harder to do if the problem is keeping more than one target alive and not focused on just one person, but it sometimes helps.
For Example situation: Titan EX - Everyone's health is low from derping and then stomps start while I'm in the middle of Medica-ing. I notice the MT's health is about 50%, so I select the MT while medica casting, then hover the cursor over Bene, just in case. Fear warranted, they're mountain bustered to about 20% HP at about the end of my cast - once medica goes off, I click that Bene immediately.
Otherwise, Bene being too slow is just something we all deal with z_z
As for the rest - healing can be very situational. The tactic you use can be determined via observation of the tank's defense habit and the DPS' output (...and ability to actually get out of AoEs).
When holying things - I really think it depends on the situation and the team makeup. I personally don't add Medica II on top of Regen since Medica II's ticks last far longer than Regen's. It has a higher chance of still ticking away by the time the mob is dead and we all know tanks are just going to running off, regardless it's still on them. ;) I have heard of people using it for that, but personally don't. Regen tends to fall off roughly at the time of the last mob's death, for most of my runs. I also wouldn't suggest using Medica II if you have a BLM thats AoEing/Flaring. Things tend to die pretty fast. A divine seal + top off cure + Regen is usually enough to sustain the tank a few holys in mobs. The first two-three holys will stun the mobs anyway, so healing shouldn't be too necessary for at least the first two. The Regen ticks should help gain back what HP was taken during your first holy.
If you have like, two melees that don't seem to be very strong in their DPS and mobs are taking a while, on top of people getting hit often, it could be an option. I just prefer not to with exception to raids where the possibility of derp is high.
If you're still insecure about the time holying and the amount of HP taken away during that cast, you can use Presence of Mind to speed up your casts.
I tend to stance dance when I'm being Holy-aggressive. Holy three times, slap Cleric off, Cure II, Slap Cleric back on, Holy two or three times, Slap Cleric off, etc. pretty much the same way as I described "queuing" Bene - I tend to have my cursor hovering Cleric when I'm Holying. Again it's situational and just paying attention to how hard the tank is being hit and forming a rhythm based on their HP loss.
With Presence of Mind, you'll get about two (or three?) Holys in before Cleric comes off CD. I've become a bit robotic about that combination, at this point.
I tend to Swiftcast + Holy for emergencies ---- for the stunning. If I feel like the party is being overwhelmed, my healing is overwhelmed and I just had to Bene the tank with plenty of mob still hanging about, I'll SC a Holy right after the Bene if the mobs haven't stun resisted (for ex, if we're fighting a mob, and we're badly positioned to suddenly accumulate more mob thats roaming).
Also works if you see mobs (or in raids, spawned trash mob... I can not honestly say I've seen many OTs go after the trash mobs that are going straight for the healers.) going straight for you and no one is picking them off. I'll also do this if I see a squishy in another alliance getting slapped by trash mob. Other healers will often help out the squishy (or help themselves) while they're being slapped, so I'll take the initiative to stun the monster(s) so theres more time for DPS/OT to get to it, the person can run away/catch up on heals or casters tear it apart/slow it (and quite honestly, I rarely see tanks pick up anything I drag to them, so I've stopped bothering in raids >_> but probably because it's hard to tell in the mess of things). It's just buying a short bit of time to survive.
SC + Holy can be a very nice self defense mechanism.
So I see Garuda is pretty easy, like you guys said, but... I wasn't able to beat it yet. Why? Tanks not knowing how to provoke. We kept getting killed because the spiny plume kept give who ever had the aggro on it 3 stacks. At one point I was sitting there, watching my stacks go up, and the tanks kind of just... kept going at Garuda/sisters. Sigh! I'll try again some other time.

I wish I had something to share. :( I mean, last night I had a group of people who refused to handle any mechanics whatsoever in Amdapor Keep (Hard), but we only wiped once because I got nabbed by the slime during the last phase of Ferdiad (though I have to admit, I have no clue how the hell the tank survived with 6 stacks of the laser debuff, Stoneskin and gimped healing can only do so much when the tank buster smacks you for 75% of your max HP). And that's hardly nightmarish compared to what others share here. D:
I shall now excuse myself and look for mirrors to break so I have interesting stories to share in Heavensward!
So once more I notice that I was not born to be a Healer.
Out in my quest to max my WHM out just for having it on 50, I was at "The Eyes Have It" ONCE MORE WOO \o/

At least this time I was not a BLM, so I could properly heal the person tanking it, except I was playing solo, never ask for party (specially because I dont wanna heal people) and my targetting isnt the most specialized for this task either... even worse with a controller...
I might say Im not much of a healer but when I see hurt people as one I tend to just heal them :/ (just dont wanna party tho).
So the FULLCIRCLETROLOLOLSWIPEOFDEATH club thingy happens and I see a dead DRG faaar away. Since other healers were dealing with the tank's HP, I say, why not? and try to res them... then first I find WHERE is that one dusty skill, had to browse my full hotbar... then found it and clicked on it, since I already had them targetted, and it failed D:
"oh Im just too far" I notice, then begin to walk to them and FULLCIRCLETROLOLOLSWIPEOFDEATH!! right beside me AAAND there goes Lein. I have no idea how that thing to to my side... it was just there by the time I realized... damned tunnel visioning...

Thanks, I have a healer friend who is very specialized in targetting with controller, really, she has told me about it a few times, but I am negatively stubborn on some stuff for some odd reason :/

Had a strange case in WOD the other day. We beat the first boss with zero issues, but then someone pulled the dragon boss too early and its aggro was bouncing all over the place. I tried to keep a few of my party members healed and I kinda fudged up on healing the tank, though the other healer also wasn't healing him. Tank proceeded to whine about getting no heals as he eats the floor. Aggro on the boss goes all over the place again and all 3 alliances slowly get killed off. People then decided to vote abandon AFTER JUST ONE WIPE and it passes.
Biggest. Waste. Of. Time.

5 headed dragon is the worst when people do that - his cleave is so wide it pretty much rips the entire raid apart easily from the get-go if he isn't spun away fast enough. I've had exactly what you described happen on several occasions (and why is it always via a DRG??). The chaos is strong in that one.
Well, on the upside, if the tank was that whiny I'd hate to see what the guy woulda been like for the rest of the raid... >_>
But still, what a pain in the arse... :/
SMNs have this pet issue especially in trials...
My guess is that the problem occurs when it's either their first duty of the day or they just switched classes.
Setting your pet to obey while having a attackable target will make your pet go attack it.
Ugh, that's me every other time I switched to SCH to level it. I usually remember after it's initial sprinkle of AoE. After that, I slap the obey button a million times and hope no one noticed. :x
I haven't seen too many issues with SMNs, though - only now and then. Then again, I rarely see SMNs in a party, as of late o_o

I was tanking and I notice this a lot:
Panic healing.
You know the type. I recognize the type when the healer spams medica 2 and cure 2 and regen all within one pull with my CD still on. Nobody was getting hurt and well yeah.
*Boss preparation (stoneskin bla bla)
*WHM casts Medica 2 and regen on me
Stahp.
*I right clicked on the HoTs to make them go away and proceed.
*OHMAGAD MY TEAMMATES NEED HEALING ASAP
*Better spam medica 2 and lose all those precious MP
*Nobody was actually getting hit except me..............................
Keep seeing this happen nowadays. Even on expert. What's going on? Suddenly all the healers get this panicking disease that would make them spam over heals all day long.
Probably people that don't main healer tbh. On SCH I try to do as little healing as possible in low end stuff like ex roulette, so I can help out with DPSing. Some of the bosses in EXR can be healed by the faerie on her own (first 2 bosses of WP HM, first boss of AK HM).
I think people are getting used to really bad groups in DF where they have to babysit the DPS on big pulls. Sometimes i just use Medica II on a big pull because it means i don't have to keep healing between holy. That said there are some Healers in denial in EXDR and it's really getting boring. Had a group yesterday in progress Healer had rage quit on first boss AKHM the Tank is pretty much minimal requirement for it which is fine we got through it with some extra care. Some Healers just expect to have an easy time every run they should just learn to carry harder instead of throwing a fit because they need to actually work.
I see that often too. And when I try to explain it, either people just remain silent (no idea why?), or they answer "I'm in lazy mode", or "mind your business". I've seen and tried to help dozens of those healers in the last couple of months : only 2 seemed interested (and asked for even more details) and were thankful. All the others ? I guess they just don't care.
When I see my co-healer casting Med2 in prepull and his ilvl being 120+, I can tell the raid/run is gonna be a nightmare. Overabuse of cures ? --> Overaggro ---> Death and all that generally leads so solo-healing while the other healer is lying in the dust..
The times I'll cast Medica II while the tank is the only one hit, at bosses = when I know a boss does room-wide AoEs soon, or does random attacks on random members. WP HM bosses for example. If the damage isn't that big after they get hit, I'll let the medica II tick it back.
I'll also do it if the party gives me reason not to trust them to help themselves (not getting out of AoEs in bosses, etc. Thats moreso in raids, though).
I used to say something to the co-healers that medica II before a pull. Only ever met with silence and the stubbornness to do same thing over again at the next boss, so now it's kind of whatever. I just sigh and hope the tank is either not MT, can override the tick on pull or it wears off before adds spawn. Now and then I'll say something and try not to come off as harsh, but I don't know if I ever get through.
Like DarkerOrange said, it's probably people that don't main WHM/healer and/or new healers watching/mimicing SCHs pre-cast Succor and not realizing the HoT effect of Medica II

We were 3 friends queing for lowbie duns, but even tho we were 1 Tank (me) and 2 DPS the timer said 5 mins wait.
It felt strange that it didnt pop instantly and said, "well, since Im just helping them it doesnt matter to me if I tank or not... so, maybe..." *Changes to WHM* *tries again* *insta pop* fml D:!!
So we get in Tam-tara. First thing my friends do is just laugh cuz they know just how much I dislike healing damn it XD (I still like being helpful so its a bit conflictive). I just hope this doesnt become an habit.
So this random Tank is of the little words but good at their stuff kind, except that he was pulling 3 groups together D:?! Wait, this is Tam-tara NM, right?! Down to ilvl 19 synch right?! We'll have to AoE!! What do we have?!
Friend 1) Archer! 1 very TP expensive AoE and has no Invigorate.
Friend 2) Pugilist! No AoE.
Me) WHM... GUESS I COULD MEDICA THEM TO DEATH!! (of course I didnt)
But seriously, I was glad the Tank stopped at a corner to ask if it was a full run or just speed it, so I managed to sneak a "please just keep it to only 3 monsters at a time since we have no AoEs" and they did listen D:! Can you believe it?
As an advance, I did manage to sneak a CS dance every now and then... but Im still sticking to poke stick my enemies if I see the chance (I did a 20 CRIT!!)
Yep, that is likely it. There are many non-main healers I've seen in casual content like WoD even wearing some Final Coil gear but with an i110 Yagrush (usually a sign they don't main with such disparate ilvl gear, and no advanced relic) and doing loony things like pre-casting medica 2 before a pull.
As SCH I usually let the fairy do most of that healing too in those EX situations, all that DET helps those fairy heals!
