I really hope SE is looking into this. It's to the point where any progression I'd like to make with friends is largely just a roll of the dice with abilities like "Plumes" on Titan if they are used/coincide with the very frequent 1-2 second heavy desyncs/packet loss or worse the less frequent 5-10 sec drops. If SE isn't actively communicating and working on solutions in regards to this subject which is affecting large masses of their game population, then I don't see a purpose for me subbing to the game, as Raid PvE Progression with friends was my reason for wanting to pick up the game.
I play from Southern California, ISP provider is Cox. I do not have any issues with any other games. Cox does not throttle bandwidth; I've run multiple tests to ensure this.
5-10 seconds of no updates to client every few minutes; frequent 1-2 second delays with desync.
This leads to really odd things happening: dot timers will literally change amount of seconds up and down; will use an on GCD ability and and off GCD abilty without the game giving me a clue that either have registered till a second later when they both register at the same time along with other players abilites used within the 1-2 second frequent desyncs; I've had Titan's plumes spawn on everyone, then ~.75 seconds later, a seperate plume spawns under me; once I competely derped a Plume and accidently used Wind bite (locking my movement into that direction for the animation) which put me back clearly into the Plume (after already being out) yet I didn't get hit (probably the 1-2 sec random packet loss/desync, client not being able to update/communicate with server that I messed up and was actually back in the Plume); there are Plumes that I tested where I would never stop running straight (in anticipation) of the Plumes being spawned and still get hit while clearly 5 feet outside of the Plume.
This is also effecting another member of our guild who has the same experience with frequent 1-2 scond spikes and occasional 5-10 second cuts, he is also using Cox Communications as a provider.

Was doing a lot better until after maintenance tonight. The Duty Finder is brutal.

Experiencing same thing as well. Rubber-band issues and R0 every single day playing.

Past 3 days after about 8-9PM EST I've experienced the same thing. When it's happening: every 30-60 seconds or so lag spike = DCing icon then its back 5 seconds later
Cablevision outside of NYC

Having this issue as well, both yesterday night and today. As mentioned, it appears to get much worse in instances. I attempted to run Wanderer's Palace today, and was very frequently experiencing freezes followed by everything fast forwarding to catch up. I had zero problems during the day, it only seems to happen at night around 8-10pm EST.
I'm on Behemoth. Location is Ohio, provider is Cincinnati Bell.
I hope this can get fixed soon, I want to be able to run instances and not bring my party down because of something I personally cannot control.
Thanks, and keep up the hard work SE! :)

I have this issue as well, on Behemoth. Lag spikes where nothing happens than it all catches up. I use Comcast and had a tech out and he found nothing so tonight I fired up SWTOR and GW2 and played both tp test it and had no issues in either game, just FFXIV
